To be cleared medically by the athletic training staff, all student-athletes and parents of student-athletes (new, returning, and transfer student-athletes) must complete and return the following forms to the Athletic Department. In addition, all athletes must have a pre-participation physical exam from Cowell Wellness Center prior to participation in any University of the Pacific athletic activity. Physical exams are arranged through the athletic training staff. Each sport will be arranged with a specific date and time in the fall of each school year. Student-athletes can obtain schedules through their coaches or by coming into the athletic training room in the Pacific Intercollegiate Athletic Building. All participating student-athletes must make their arranged physical dates. Student-athletes need to send forms by July 15, 2008 to:
